I am an advocate lawyer in East Stroudsburg, PA working on behalf of many clients with mental health impairments of varying degrees. I am looking for a mental health professional to whom I may refer many of my clients in need.
Many of my clients are receiving medical assistance / Medicaid. Thus, I am looking for a skilled, compassionate, licensed mental health counselor / therapist in PA and/or NJ who can meet the following:
accept Medicaid patients/clients, and
agree to provide the advocate full and thorough mental health treatment records, upon receipt of proper HIPAA compliant release, in a timely manner and at reasonable reproduction cost.1
If you are interested in my office referring to you clients in need of ongoing mental health counseling/therapy, and providing them the above professional treatment, recordkeeping and records access, please call me at 201-317-0610 or email me at firstname.lastname@example.org.
Thank you for your time. I look forward to hearing back from you.

It is acceptable to refrain from providing such notes directly to the patient/client if same would be detrimental to his/her health, but in this arrangement, mental health professional must agree to provide all such records to the advocate attorney of the patient/client, or the agency that the patient/client is using an advocate for (such as the Social Security Administration, or similar).
